About the Role
The role involves providing clinical social work services remotely to individuals in need of behavioral health support, working closely with primary care providers to ensure holistic patient care.
Responsibilities
- Conduct remote clinical assessments to evaluate patient mental health needs
- Deliver evidence-based psychotherapy to individuals and families
- Collaborate with primary care providers to coordinate patient care
- Develop and implement individualized treatment plans
- Maintain accurate and timely electronic health records
- Provide crisis intervention and risk assessment services
- Engage patients in treatment through motivational interviewing techniques
- Monitor patient progress and adjust interventions as needed
- Participate in team meetings and case reviews
- Ensure compliance with all clinical and regulatory standards
- Offer brief, solution-focused therapy within primary care settings
- Support patients in managing anxiety, depression, and life stressors
- Facilitate patient referrals to appropriate community resources
- Promote mental health awareness and reduce stigma
- Use telehealth platforms to deliver secure and effective care
- Adhere to confidentiality and HIPAA requirements
- Respond promptly to patient messages and scheduling requests
- Participate in quality improvement initiatives
- Stay current with best practices in behavioral health
- Contribute to a patient-centered, trauma-informed care environment
