About the Role
The associate works closely with clinical and operational teams to enhance patient experience through proactive outreach, scheduling support, and care plan follow-up, ensuring families remain engaged throughout the treatment process.
Responsibilities
- Conduct regular check-ins with patients and caregivers to assess needs and satisfaction
- Coordinate appointment scheduling between families, clinicians, and therapists
- Update and maintain electronic health records with current patient engagement data
- Respond to incoming calls and messages from families in a timely manner
- Assist in onboarding new patients by guiding them through initial processes
- Track patient progress related to care plan participation and follow-up tasks
- Escalate clinical or urgent concerns to appropriate care team members
- Support care transitions by providing resources and logistical information
- Verify insurance and service eligibility in coordination with billing staff
- Collaborate with supervisors to identify gaps in patient engagement
- Participate in team meetings to review patient case updates
- Ensure compliance with privacy and confidentiality standards
- Follow up after patient visits to confirm understanding of next steps
- Distribute educational materials tailored to individual patient needs
- Monitor outreach efforts and document outcomes in tracking systems
- Assist with care coordination for multidisciplinary therapy services
- Maintain consistent communication with external referral sources
- Help resolve non-clinical barriers to treatment adherence
- Support quality improvement initiatives through data reporting
- Use CRM tools to log interactions and schedule future touchpoints
