Harrisonburg, VA Work Shift: First (Days) Overview: Sentara RMH Medical Center is recruiting an Inpatient Case Management Registered Nurse! Inpatient Case Manager Registered Nurse Sentara RMH Medical Center - Harrisonburg, VA ~Full Time & Day shift~ 40 hours/week Monday-Friday: 8:00am-4:30pm, Rotating weekend (every 6 weeks) Sentara RMH Medical Center is currently seeking an Inpatient Case Manager. This role is responsible and accountable for delivering comprehensive care management, services, and quality outcomes across the healthcare continuum, with a primary emphasis on guiding patient transitions to appropriate care levels while ensuring clinical effectiveness and optimal length of stay. Overview Accountable for comprehensive care management, services, and quality outcomes throughout the healthcare continuum, focusing on patient transition to optimal care levels while maintaining clinical appropriateness and length of stay. Provides direct support to in-hospital patients requiring extensive case management. Promotes effective healthcare service utilization and monitoring, collaborates with healthcare teams and patients/caregivers to manage care and transitions. Develops and implements comprehensive care plans based on thorough assessments addressing physical, mental, and psychosocial patient/caregiver needs. Operates exclusively in hospital settings. BLS certification required within 90 days of hiring. BSN preferred. Case Management certification (ACM, CCM, CCCTM, CMAC, or CMGT-BC) mandatory within one year of eligibility. One-year acute care case management experience preferred.
