Responsibilities
- Review case load and prepare technology at the start of the day to ensure all tools are available for patient service
- Navigate daily schedule using modern technology and partner with in-home clinical team members (Sprinters) who visit patients’ homes
- Create warm and welcoming connections with a diverse range of patients to collect relevant information and perform various services
- Conduct thorough health risk assessments, medication reviews, cognitive screenings, and provide educational information on health and well-being
- Engage in proactive care through virtual wellness visits for adults and/or pediatric patients
- Work directly with Sprinters to evaluate vital signs, arrange blood draws, and carry out necessary patient-specific tasks
- Provide comprehensive care coordination and management including preventive care interventions, medication management, referrals to specialists, and community resources
- Document findings accurately and securely
Requirements
- Board Certified as a Family Nurse Practitioner
- Credentialed Medicaid provider for the state of Kentucky
- Active Family Nurse Practitioner License in Kentucky
- Consistently exhibits the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion to provide high quality health services
- Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking
- Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Pass national background check and valid clinical license search
