Deliver patient-centered care from anywhere as a Family Nurse Practitioner with a mission-driven organization transforming how primary care is delivered. You'll conduct virtual wellness visits, connecting directly with patients in their homes while collaborating with in-home clinical team members who support on-site assessments.
What You’ll Do
- Begin each day by reviewing your schedule and preparing digital tools for seamless virtual consultations
- Build trusting relationships with patients across a variety of backgrounds and health statuses
- Perform comprehensive health risk evaluations, medication reviews, and cognitive screenings
- Collect essential health information and guide patients through preventive care strategies
- Coordinate with in-home clinicians to assess vital signs and arrange lab work, including blood draws
- Educate patients on managing their health and connect them with appropriate specialists or community programs
- Ensure accurate documentation of all clinical findings using secure charting systems
- Support care coordination and long-term management plans tailored to individual needs
What We’re Looking For
- Current Family Nurse Practitioner licensure in GA, NC, SC, FL, or MS
- Board certification as a Family Nurse Practitioner
- Proficiency with laptops, mobile devices, and digital health platforms
- Experience using HIPAA-compliant messaging for clinical coordination
- Strong communication skills and the ability to work independently with minimal supervision
- Commitment to confidentiality, professionalism, and ethical patient care
- Adaptability in fast-evolving situations requiring creative problem-solving
- Pass national background and license verification checks
