Sprinter Health is seeking a Family Nurse Practitioner licensed in California to provide virtual wellness visits to patients in their homes. The role involves conducting health assessments, medication reviews, cognitive screenings, and coordinating care with in-home clinical team members while leveraging technology to deliver high-quality, patient-centered care.
What You'll Do
- Review case load and prepare technology at the start of the day
- Navigate daily schedule using modern technology
- Partner with in-home clinical team members (Sprinters) who visit patients' homes
- Create warm and welcoming connections with a diverse range of patients
- Collect relevant patient information and perform various services
- Conduct thorough health risk assessments
- Perform medication reviews
- Conduct cognitive screenings
- Provide educational information to patients about their health and well-being
- Work directly with Sprinters to evaluate vital signs
- Arrange blood draws
- Carry out necessary tasks to address specific patient needs
- Provide comprehensive care coordination and management
- Deliver preventive care interventions
- Manage medications
- Refer patients to specialists
- Connect patients with community resources
- Document findings accurately and thoroughly
What We're Looking For
- Board Certified as a Family Nurse Practitioner in California
- Active Family Nurse Practitioner License in California
- Consistently exhibits the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion
- Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking
- Technologically savvy and comfortable using tools such as laptops or mobile devices for charting
- Comfortable using HIPAA secure messaging apps for care coordination
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Pass national background check
