Responsibilities
- Provide case management and oversight for youth in certified therapeutic foster homes
- Maintain weekly contractual contact with youth on assigned caseload
- Support Family Care Providers with guidance, coaching, and problem-solving
- Participate in service planning, assessments, and skill development activities
- Address behavioral challenges and support placement stability
- Collaborate with multidisciplinary teams to coordinate services
- Maintain accurate, timely, and compliant documentation
- Participate in after-hours and weekend on-call rotation as scheduled
Requirements
- One of the following: – Bachelor’s degree in Social Services or a related field with at least two (2) years of experience working with at-risk youth or young adults – Graduate degree with a minimum of one (1) year of relevant experience
- Valid Driver’s License, clean driving record, and active auto insurance
- Ability to pass state and federal background checks
