As a Condition Management Account Coordinator, you will serve as a primary point of contact, helping participants navigate program resources, understand their care plans, and overcome barriers to engagement. You’ll combine empathy with strong organizational skills to deliver personalized service while accurately documenting interactions and following up promptly. Success in this role means improving participant satisfaction, increasing program adherence, and contributing to better health outcomes—all within a collaborative, supportive team environment that values continuous improvement and participant-centered care.
Responsibilities
- Respond to participant questions and provide assistance through inbound and outbound calls
- Support participants using email and online messaging platforms
