Lola Blankets is hiring a Complex Care Nurse Practitioner/Physician Assistant to join our team providing primary care to homebound and complex care patients across Philadelphia County. On a full-time basis, you will apply medical techniques under physician supervision to care for a diverse panel of patients with chronic diseases.

What You'll Do

• Provide high quality primary and palliative care to patients in and around Philadelphia, PA.
• Assess patient health by performing physical examinations and obtaining, updating, and studying medical histories to create optimal care plans.
• Effectively communicate with office staff and clinical operations across various internal platforms.
• Provide high quality, patient-centered care to your assigned panel.
• Develop clinical impressions, diagnoses, and treatment plans.
• Foster trust and consult with patients, families, and caregivers during visits.
• Order medically necessary diagnostic testing.
• Prescribe medications, supplies, and other appropriate medically necessary interventions.

What We're Looking For

• Board Certified, with a valid NP or PA certification in the state of Pennsylvania.
• Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose.
• Positive demeanor and strong people skills.
• Ability to confidently problem solve and operate independently.

Nice to Have

• 1-2 years of primary care experience with geriatric patients.
• Hospice experience.

• Compensation range: $105,000 - $140,000+ depending on experience and other incentives.
