About the Role

Mass General Brigham is hiring a Community Health Worker II to deliver advanced health education, outreach, and care coordination to individuals and families. In this role, you will collaborate closely with healthcare providers to ensure patients receive appropriate care and support services, directly improving community health outcomes.

What You'll Do

  • Participate in the weekly BH pediatric triage meeting to review referred patients needing in-home therapy services.
  • Remain familiar with family and IHT programs to complete referral paperwork and navigate families’ transition to IHT.
  • Attend the monthly community Systems of Care Meeting and other recommended care collaboration meetings.
  • Utilize the IRIS referral system to routinely document local referrals and patient participation in other agencies.
  • Stay current with local updates in community programs and newly established local resources.
  • Conduct in-depth assessments of patient needs, goals, and barriers to achieving good health outcomes.
  • Provide advanced health education and coaching to promote healthy behaviors and self-management of chronic conditions.
  • Collaborate with healthcare providers to develop and implement care plans for patients with complex health needs.
  • Assist patients in navigating the healthcare system and accessing services, including health insurance and social services.
  • Provide ongoing follow-up and support to ensure continuity of care and successful achievement of health goals.
  • Develop and implement health promotion programs and activities to address community health needs.
  • Collect data and maintain accurate records of patient interactions and outcomes.
  • Attend meetings and trainings related to community health promotion and education.

What We're Looking For

  • High School diploma or GED.
  • 2-3 years of experience in community health outreach, health education, or a related field.
  • Demonstrated ability to work effectively and provide advocacy for all populations and communities.
  • Strong communication and interpersonal skills, with the ability to interact effectively with various populations.
  • Ability to work independently and as part of a team.
  • Basic computer skills, including Microsoft Office and database management.

Nice to Have

  • Bachelor's Degree.
  • Community Health Worker (CHW) license from the Massachusetts Board of Allied Health Professionals.
  • Bilingual skills.
