About the Role

Responsibilities
- Engage with community members to identify health and social service needs
- Provide information about available medical, behavioral, and preventive care options
- Assist individuals in navigating healthcare systems and scheduling appointments
- Build relationships with local organizations to expand resource accessibility
- Conduct home and community visits to support care plan adherence
- Track participant progress using digital tools and case management platforms
- Facilitate group discussions on wellness, prevention, and chronic disease management
- Coordinate with care teams to ensure holistic support for individuals
- Promote culturally sensitive communication in all interactions
- Report community trends and barriers to care leadership
- Support crisis intervention by connecting individuals to emergency services
- Encourage participation in health education programs
- Maintain confidentiality and compliance with privacy regulations
- Participate in ongoing training on public health topics
- Respond to participant inquiries via phone, text, or in person
- Assist with transportation or access challenges to medical visits
- Document interactions and follow-ups accurately and promptly
- Work evenings and weekends as needed to meet community needs
- Represent the program at local events and outreach activities
- Use mobile technology to update records and communicate with teams
- Support language access by coordinating interpreter services when necessary
- Identify gaps in community resources and suggest improvements
- Follow up with participants to ensure continuity of care
- Uphold principles of equity and inclusion in all engagements
- Adapt communication style to meet diverse literacy and cultural needs
Nice to Have
- Certification in community health work
- Bilingual proficiency in Spanish and English
- Experience with mental health or substance use support
- Training in conflict resolution or de-escalation
- Familiarity with Medicaid or public insurance systems
