The Oregon Department of Human Services is hiring a Clinical Pharmacy Practitioner (Pharmacist) at the Eastern Oregon Correctional Institution in Pendleton. In this critical role, you will work to improve patient care and reduce pharmaceutical spending, acting as a key resource for providers, nurses, and pharmacists.
What You'll Do
- Work to improve patient care and reduce pharmaceutical spending.
- Act as a resource for providers, nurses, and pharmacists on challenging pharmaceutical questions.
- Participate in a Collaborative Therapy Management (CTM) program as a mid-level practitioner providing direct patient care.
- Train nursing staff and providers on pharmacy software systems.
- Assist in maintaining a working formulary and review new pharmaceuticals.
- Provide regular pharmaceutically aligned meetings to improve staff knowledge.
- Review patient treatments regarding polypharmacy and medication therapy management (MTM).
- Assist in treatment plans and attend Therapeutic Level of Care (TLC) committees as a drug expert.
- Provide advice and consultation to physicians and other direct care staff.
- Provide information to adults in custody (AICs) to increase understanding and compliance.
- Engage with outside providers and specialists to procure specialized pharmaceuticals.
What We're Looking For
- A current Pharmacist's license.
- Ability to obtain an Oregon pharmacist license issued from Oregon State Board of Pharmacy prior to hire.
- Three years of professional experience related to the class concept.
Nice to Have
- Three years of professional experience in a patient-oriented system.
- Graduation from an accredited School of Pharmacy.
- State of Oregon Pharmacist License, upon hire.
- Computer experience – MS Office Suite, Pyxis.
- Clinical experience in application of evidence-based guidelines.
- Board Certification (BCPS, BCACP or similar).
- Medication Therapy Management (MTM) experience.
- Experience working in a correctional facility or jail.
