A clinical pharmacist supporting remote medication management and quality improvement initiatives through telephonic patient engagement and collaboration with care teams.
Responsibilities
- Provide telephonic counseling to patients on medication adherence and resolve medication-related issues including non-adherence, polypharmacy, and use of high-risk drugs, offering evidence-based recommendations to patients and care teams.
- Conduct targeted clinical chart reviews when necessary to guide patient-specific interventions.
- Document patient commitments to medication adherence and address obstacles to compliance during real-time interactions.
- Refer complex clinical cases to prescribers or interdisciplinary teams as appropriate for further management.
- Achieve and maintain performance benchmarks related to quality measures such as STARs and HEDIS.
- Work with physicians and clinical and administrative staff to enhance medication management processes and improve patient care experience.
- Identify medication management gaps through patient conversations and convert insights into actionable recommendations for improving clinical workflows and practice operations.
- Engage in the development, execution, and assessment of pilot initiatives or process improvements within the outreach framework.
- Act as a clinical pharmacy expert, applying specialized knowledge to support organizational goals and respond to changing healthcare demands.
- Create and update standardized clinical resources, educational materials, and protocols to improve medication outcomes and support quality performance for clinicians, practices, and patients.
- Perform additional duties as required.
Requirements
- Doctor of Pharmacy (PharmD) degree required.
- Must hold an active, unrestricted pharmacist license in the state of residence and maintain valid multi-state licensure in at least two of the following states: Alabama, California, Delaware, Kentucky, Louisiana, Mississippi, North Carolina, Ohio, Pennsylvania, Utah, Virginia, or West Virginia.
- Willingness to obtain and keep additional pharmacist licenses within six months of hire.
- Minimum of three years of professional experience in value-based care, ambulatory care, or community pharmacy settings.
- Comprehensive understanding of pharmacotherapy and pharmacy practice standards.
- Strong communication skills, both verbal and written, with the ability to clearly explain complex clinical information to patients and care teams.
- Proficient in using telephony systems and navigating various Electronic Health Record (EHR) platforms.
- Capable of thriving in a fast-paced environment and adapting quickly to evolving priorities.
- Availability to work between 8 AM and 7 PM Eastern Time as required by operational needs.
Nice to Have
- Experience or familiarity with Medicare, HEDIS, and STAR ratings is preferred.

Additional Information
