Healthmap Solutions is hiring a Clinical Nurse Liaison to champion better health outcomes for our Kidney Management Program members. In this role, you will develop and maintain long-term partnerships with physicians, their office staff, and our members to coordinate performance improvement and care management activities.
What You'll Do
- Act as a key liaison between Healthmap, provider practices, and members to ensure positive program engagement.
- Develop new provider relationships and manage your assigned caseload to improve member health outcomes.
- Identify and act on opportunities for improvement using provider-specific data.
- Educate providers on Best Practices, process improvement, Kidney Health Management interventions, and HEDIS/STAR measures.
- Provide education on Chronic Kidney Disease, End Stage Renal Disease, and Renal Replacement Therapies.
- Partner with physicians and their staff to identify members for Care Navigation support and conduct member outreach.
- Educate Healthmap members on kidney health, co-morbid conditions, and renal replacement therapy options.
- Advocate for members, connect them with community resources, and serve as a liaison between members, their support network, and providers.
- Be accountable for individual and departmental metrics and key performance indicators.
- Ensure timely delivery of reports to internal and external stakeholders.
- Maintain thorough documentation of all provider and member interactions in compliance with NCQA standards.
- Adhere to Healthmap policies and procedures and comply with HIPAA and all other federal, state, and local regulations.
What We're Looking For
- Bachelor's degree in Nursing.
- Active, unrestricted RN license.
- Basic Life Support (BLS) certification.
- Advanced Cardiovascular Life Support (ACLS) certification (role-dependent).
- 3+ years of progressive experience in healthcare services, clinical operations, quality, or care management.
- Valid and unrestricted driver's license.
- Must reside in your assigned state.
- Compliance with organization health screening and immunization policies, including TB test/chest X-ray and proof of Hepatitis B, MMR, Varicella, COVID-19, and Influenza immunizations.
- Participation in annual health and wellness screenings.
Nice to Have
- 3 years of experience in care gap closure or care coordination activities, ideally in an outpatient or hospital setting.
- Prior experience building and managing relationships with healthcare providers or patients.

- Salary range: $105,000 - $138,000 (dependent on specific market/region and candidate experience).
