Bayshore HealthCare is seeking a Client Service Coordinator (Healthcare Scheduler) for a 12 to up to 18-month maternity leave coverage position at our Hamilton Home Care Solutions Branch. You will serve as the vital link between patients, families, and healthcare providers, focusing on scheduling and coordinating home care services.
What You'll Do
- Professionally engage with patients, families, staff, and funder organizations.
- Quickly handle funder referrals and updates in the EMR.
- Collaborate with Supervisors, field employees, and patients to assess service needs.
- Assign care providers based on client needs, geographic area, staff skill-set, and availability.
- Ensure consistent caregiver assignments and service delivery.
- Keep patients and staff informed of initial and ongoing schedules.
- Optimize staff schedules to meet their availability and client care needs.
- Maintain accurate records in the EMR.
- Address patient concerns promptly and follow escalation pathways.
