Guaranteed Health is looking for a Case Manager to support members in our MediCal programs in San Jose and Gilroy. You will be a key player in our mission to change the death and dying experience by engaging members and navigating the healthcare system to ensure seamless, excellent care.
What You'll Do
- Identify and engage potential members eligible for Guaranteed's services.
- Collaborate with hospitals, nursing homes, churches, community organizations, and doctor offices to source referrals.
- Educate case managers and healthcare professionals about eligibility criteria and referral processes.
- Serve as a liaison between our organization and community partners.
- Build and maintain strong relationships with healthcare providers, community leaders, and organizations.
- Represent the organization at community events, meetings, and forums.
- Provide recurring care coordination for members.
- Facilitate referrals to community support services based on members' needs.
- Coordinate care plans with patients' primary care physicians (PCPs) and other healthcare providers.
- Monitor patient progress and adjust care plans as necessary.
- Maintain accurate and confidential records of patient interactions and care plans.
- Prepare regular reports on outreach activities, patient recruitment, and care coordination outcomes.
What We're Looking For
- Proven experience as an in-person marketer or community liaison.
- Demonstrated ability to work effectively with community partners, healthcare professionals, patients, and families.
- Understanding of and sensitivity to multi-cultural communities.
- Valid California Driver's license, valid automobile insurance, and a clean driving record.
- Willingness to work Monday-Friday 8:30am-5:00pm, primarily in the field.
Nice to Have
- Bilingual in Spanish.
Benefits & Compensation
- Compensation: $25-$32 per hour depending on experience.

This is a local, field-based position in Gilroy, California.
