Stanford Health Care is hiring a CASE MANAGER (RN) - Transitional Care / Adult Aging Services - Full-time (1.0 FTE), 8-hr Days

About the Role

Stanford Health Care is seeking a Case Manager (RN) specializing in Transitional Care and Adult Aging Services. In this role, you will coordinate the continuum of care and discharge planning for a caseload of patients, focusing on the aging population. As part of an interdisciplinary team, you will ensure timely discharge, appropriate resource utilization, and safe care transitions from hospital to community.

What You'll Do

  • Coordinate all facets of a patient's admission and discharge, managing their transition through the healthcare system.
  • Perform utilization review activities, including reviewing patient charts for timeliness and appropriateness of services.
  • Coordinate and facilitate timely implementation of discharge plans for patients with complex needs.
  • Act as an educational resource and provide consultation to patients, families, and hospital staff regarding discharge planning and regulatory requirements.
  • Participate in program development, quality improvement initiatives, and department committees.
  • Review patients for appropriateness of admission, level of care, and length of stay; implement strategies to reduce length of stay and resource consumption.
  • Collect, analyze, and report data for utilization, quality improvement, and compliance.
  • Collaborate with third-party payers to assure reimbursement for patient services.

What We're Looking For

  • A Bachelor's Degree from an accredited college or university.
  • A current Registered Nurse (RN) state licensure or compact state licensure.

Nice to Have

  • Three (3) years of progressively responsible and directly related work experience.

Team & Environment

You will work within an interdisciplinary team which includes an Advanced Practice Provider, Social Workers, Gerontologist, and Occupational Therapist.

Benefits & Compensation

  • Compensation generally starts at $72.55 - $96.15 per hour.
  • Benefits begin the first day of the month following employment eligibility.
