CVS Health is looking for a Case Manager RN to join our team. This 100% remote position allows you to work from any state. You will be at the forefront of transforming healthcare, utilizing a collaborative process of assessment, planning, and advocacy to meet members' health needs and promote optimal, cost-effective outcomes.
What You'll Do
- Apply data-driven methods to identify members and fashion individualized case management programs.
- Conduct comprehensive clinical assessments.
- Evaluate needs and develop flexible approaches based on member needs, benefit plans, or external programs.
- Advocate for patients to the full extent of existing health care coverage.
- Promote quality, cost-effective outcomes and suggest improvements for program efficiency.
- Identify and escalate quality of care issues through established channels.
- Provide a very high level of customer service.
- Utilize assessment techniques to determine member's health literacy, technology capabilities, and readiness to change.
- Use influencing and motivational interviewing skills to maximize member engagement and promote lifestyle changes.
- Provide coaching, information, and support to empower members to make independent medical and healthy lifestyle choices.
What We're Looking For
- Must have an active, current, and unrestricted RN license in your state of residence.
- Willingness and ability to obtain additional state licenses upon hire (paid for by the company).
- 3+ years of acute care experience as an RN required; Med Surg or Critical Care preferred.
- Must be willing to work Monday through Friday, 9:00am through 5:30pm in your time zone, with an occasional late shift rotation per business needs (11:30am-8pm EST).
- Associate Degree required.
Nice to Have
- Compact RN License or Multi-state licensure.
- Certified Case Manager (CCM).
- Telephonic case management and/or telephonic experience.
- BSN preferred.
