Aristo Sourcing is hiring a Case Manager to play a central role in patient care. You will coordinate care for patients facing acute and chronic health challenges, serving as the primary point of connection between patients, families, and clinical teams. Your mission is to guide patients through their healthcare journeys with empathy, clarity, and professionalism.
What You'll Do
- Provide care coordination and case management services for patients with acute and chronic conditions.
- Serve as the primary point of contact for patients, families, and multidisciplinary clinical teams.
- Support, educate, and advocate for patients, ensuring their individual care needs are met.
- Coordinate care plans, monitor patient progress, and encourage adherence to treatment regimens.
- Utilize EMR/case-management software to document and monitor patient care.
- Collaborate with healthcare providers and insurance companies as needed.

What We're Looking For
- Bachelor’s degree in Nursing, Social Work, Healthcare Management, or a related field.
- Experience in customer service, care coordination, or case management.
- Strong communication and collaboration skills.
Nice to Have
- Active RN license (for nursing professionals).
- Case Management Certification (CCM).
- Proficiency with EMR and/or case management software.
