Sailor Health is hiring a Care Navigator to be the first voice a patient or their family hears. This full-time remote role guides older adults and caregivers through the intake process with warmth and clarity, blending patient support, care coordination, and emotional guidance. You will help redefine what it means to age with dignity, connection, and mental wellness.
What You'll Do
- Be the first point of contact for patients and their families reaching out to Sailor Health.
- Guide older adults and their caregivers through the intake process.
- Match patients with the right therapist.
- Help each new patient navigate the journey into care, ensuring they feel confident and understood.

Nice to Have
- Experience in healthcare, customer service, or an administrative support role.
