CenterWell Pharmacy, a Humana company, is hiring a Care Manager, Telephonic Nurse 2. We are looking for a Registered Nurse with a Bachelor of Science in Nursing to assess and evaluate members with complex health conditions via telephone, ensuring optimal wellness. This role involves developing care plans, monitoring progress, and providing medication education within a caring community focused on putting health first.
What You'll Do
- Assess and evaluate member conditions with emphasis on medication therapy through telephonic interactions.
- Develop and implement comprehensive care plans for patients.
- Monitor patient progress through regular assessments.
- Utilize advanced telecommunication systems to provide medication education.
- Adhere to a set daily schedule with inbound and outbound tasks.
What We're Looking For
- Bachelor of Science in Nursing (BSN).
- 3-5 years of clinical acute care experience.
- Licensed Registered Nurse (RN) in the appropriate state with no disciplinary action.
- Must live in OH, KY, FL, AZ, TX.
- Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook.
- Managed care experience.
- Must be passionate about contributing to an organization focused on continuously improving consumer experiences.
- Schedule Adherence.
Nice to Have
- Experience with care management, patient education for adult care, auto dialer experience.
- Bilingual English/Spanish.
