About the Role

Rockefeller Philanthropy Advisors is hiring a Care Coordinator for a mission-driven healthcare team dedicated to transforming kidney care through holistic, patient-centered support. In this role, you will be on the front lines of service delivery, engaging patients, addressing social determinants of health, and ensuring seamless care transitions.

What You'll Do

  • Conduct telephonic outreach to enroll patients into care programs and build trusting relationships as their main point of contact.
  • Collaborate with Nurse Care Managers to develop and implement personalized care plans tailored to patient needs.
  • Coordinate care transitions across healthcare settings and dialysis centers, ensuring continuity and strong social support.
  • Assess and address Social Determinants of Health, connecting patients to vital services such as housing, transportation, and food support.
  • Navigate patients through health systems and community-based resources, removing barriers to care.
  • Advocate for patients, identifying and addressing inequities and ensuring timely interventions.
  • Maintain proactive, clear communication with patients, caregivers, and clinical teams to ensure aligned care delivery.
  • Accurately document all interactions, assessments, and updates in compliance with policies and regulations.
  • Educate patients and their families to empower them in managing their health conditions.
  • Support other teams and regions as needed through cross-market coverage.

What We're Looking For

  • Minimum of 3 years of experience in healthcare coordination, community health, or a related field.
  • Proven success working in a collaborative, team-based care model.
  • Strong knowledge of local and regional community health resources.
  • Excellent communication and prioritization skills; ability to multitask and remain organized.
  • Self-starter with a solution-oriented mindset and professional demeanor.
  • Proficient in Microsoft Outlook, Teams, and other Office Suite tools.
  • Ability to work remotely with reliable high-speed internet (minimum 25 Mbps download, 10 Mbps upload).
  • Dedicated and interruption-free work environment to maintain patient data confidentiality.
