Responsibilities
- Engage with patients via text and phone to support program participation, send reminders, and monitor care progress.
- Foster high levels of patient engagement and satisfaction through consistent support and health education.
- Keep electronic health records up to date and share care plans with referring clinic partners.
- Manage a collaborative care registry to monitor patient follow-ups and track clinical outcomes.
- Generate and submit regular progress reports within the electronic health record system.
- Arrange external referrals for community social services when clinically necessary, such as housing, employment support, mental health care, or substance use treatment.
- Draft and deliver timely responses to patient and provider inquiries.
- Provide regular updates to the Lead Care Manager on patient engagement and treatment outcomes.
- Work closely with OB clinic staff to ensure seamless, coordinated patient care.
