We are hiring a Care Coordinator to serve as the essential bridge between providers, technology, and our members. In this role, you will manage the end-to-end client journey, ensuring every member feels seen, heard, and cared for.
What You'll Do
- Act as the primary point of contact for new and existing members, guiding them through onboarding and beyond.
- Coordinate scheduling between members and the clinical team, managing complex calendars.
- Help members understand their benefits and connect them with digital resources for mental health goals.
- Maintain meticulous records in CRM/EHR systems to ensure the clinical team has the best possible insights for care.
- Proactively identify barriers to care and resolve them with empathy and speed.
What We're Looking For
- 2+ years of experience in healthcare administration, case management, or high-touch customer success.
- Excellent verbal and written communication skills.
- Bilingual proficiency in English and Spanish.
- Digitally agile with high comfort switching between multiple platforms and communication tools.
Nice to Have
- Previous experience in the American health care industry.
