About the Role
The ideal candidate will help patients navigate the healthcare system by coordinating services, monitoring progress, and advocating for patient needs across multiple settings.
Responsibilities
- Monitor patient progress through regular check-ins and assessments
- Develop individualized care plans in collaboration with healthcare professionals
- Connect patients with community resources and medical services
- Track and document patient interactions using electronic health records
- Coordinate transitions between care settings such as hospital to home
- Advocate for patient needs within the healthcare system
- Identify barriers to care and implement strategies to address them
- Maintain accurate and up-to-date patient records
- Communicate effectively with patients, families, and providers
- Support patients in understanding treatment plans and medication regimens
- Assist in managing chronic conditions through proactive outreach
- Ensure compliance with privacy and confidentiality standards
- Participate in team meetings and case reviews
- Respond promptly to urgent patient concerns
- Facilitate access to mental health and social services
- Educate patients on self-management techniques
- Collaborate with insurance providers to verify coverage
- Follow up on missed appointments and unresolved referrals
- Contribute to quality improvement initiatives
- Adapt care strategies based on patient feedback and outcomes
Nice to Have
- Certified Care Manager (CCM) credential
- Experience with telehealth platforms
- Background in public health or care navigation programs
- Fluency in a second language
- Training in motivational interviewing techniques
