Community Health Network is looking for a Behavioral Health Therapist Acute to provide direct patient care through comprehensive assessment, treatment planning, and therapy. For over 60 years, our mission has been to provide exceptional care to our community, and you will be integral to that effort, focusing on safety, family functioning, and discharge planning.

What You'll Do

Provide comprehensive psychosocial assessments.
Develop individualized treatment plans focused on improving function in home, school, workplace, or community settings.
Deliver effective mental health interventions, including individual, family, and group therapies, as well as crisis intervention.
Establish and document clear links to other treatment programs within the continuum as needed.
Develop and implement efficient treatment discharge plans appropriate to client needs.
Ensure appropriate and cost-effective utilization of material and human resources.
Collaborate with a multi-disciplinary treatment team on assessment, referrals, and discharge planning.
Participate actively in professional development and education.
Engage in service line quality improvement efforts.
Comply with all applicable Behavioral Care Services policies and procedures.
Participate in Aggressive Behavior Management appropriate to the population served.

What We're Looking For

License eligibility for LCSW, LMHC, LMFT, or LCAC in the State of Indiana.
Master’s degree from an accredited college or university in a mental health related field.
2+ years of experience providing mental health treatment.
Ability to collaborate with others in a team setting.
Excellent communication skills.
Positive attitude toward problem-solving.
Commitment to the critical importance of customer satisfaction for both internal and external customers.
