About the Role
Serve as the first point of contact for incoming patient referrals and inquiries. Quickly assess each call to determine appropriate next steps. Gather accurate and relevant information during each interaction. Guide conversations toward clear and defined outcomes. Remain calm and focused in high-pressure situations. Communicate with directness and empathy. Adapt tone and approach when speaking with individuals in crisis. Interact professionally with hospital discharge planners and other referral sources. Use structured thinking to manage fast-paced call volume. Maintain composure while handling sensitive topics
Responsibilities
- Answer a high volume of calls from prospective patients and referral partners
- Conduct initial screening and intake assessments over the phone
- Collect and document essential patient information accurately
- Determine patient eligibility and appropriate level of care
- Coordinate next steps with internal care teams
- Follow established protocols for crisis-sensitive conversations
- Maintain consistent communication standards across all interactions
- Escalate complex cases according to procedure
- Ensure timely follow-up on all referrals
Requirements
- Must be located in the United States
- Must have prior remote work experience
- Must have experience working with U.S.-based organizations
- Must complete the full application form

- Proven ability to manage high-call-volume environments
