Lehigh Valley Health Network, a Magnet-recognized hospital ranked among the ‘Best Hospitals’ by U.S. News & World Report, is hiring an Access Center Triage RN. In this role, you will provide essential telephone triage services to quickly identify patients with high-risk conditions, facilitate care coordination and virtual monitoring, and direct callers to the appropriate level of care using clinical assessment skills and approved protocols.
What You'll Do
- Direct callers to the appropriate level of care and provide healthcare advice and education using clinical assessment skills, approved guidelines, and knowledge of self-care practices.
- Perform a rapid evaluation of patient situations to estimate the severity of illness or injury, including chief complaint, vital sign, and mental status information.
- Provide clinical support in symptom assessment, determining symptom acuity, offering information for symptom management, and delivering patient education.
- Facilitate coordination of care across settings for designated patients, ensuring labs, studies, and referrals are completed and results are communicated.
- Serve as a liaison between patients, physicians, the public, and hospital staff, programs, and resources.
- Refill patient prescriptions and place orders for appropriate testing or labs.
- Assist in medical record maintenance and accurately file patient charts and reports.
- Prepare and transmit documentation and medical information.
What We're Looking For
- An Associate’s Degree in Nursing.
- 5 years of nursing experience in acute care, ED, medical office, or telephone triage.
- 2 years of previous clinical acute care hospital or urgent care experience.
- Current RN license in the state of Pennsylvania.
- American Heart Association Basic Life Support certification for Pennsylvania.
- Excellent computer skills and demonstrated empathy for individuals seeking healthcare information.
- Flexibility to perform a variety of tasks in a fast-paced environment.
- Knowledge of appropriate home care information and telephone-based clinical assessment techniques.
- Skill in making triage decisions and responding calmly in emergencies.
- Ability to exercise judgment with sensitive, confidential information.
Nice to Have
- A Bachelor’s Degree in Nursing (BSN).
Work Mode
This position is remote and requires the candidate to be located within the state of Pennsylvania.
