Highmark Health and Allegheny Health Network are hiring a Wellness Coach to take primary ownership of a specified panel of members with varied health statuses and clinical needs. You will utilize data, analytics, and professional clinical judgment to assess needs and identify the right lifestyle and clinical interventions for each individual.
What You'll Do
- Maintain knowledge of evidence-based interventions to deliver high-quality lifestyle coaching across areas including tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity, and maternal health.
- Perform ongoing assessment of your assigned member panel to identify clinical needs, determine appropriate interventions, and triage members to additional resources.
- Create and manage personalized care plans for your caseload, addressing identified needs, removing barriers to care, and setting both long- and short-term goals.
- Demonstrate clinical outcomes and cost savings from wellness interventions while supporting members across all demographics.
- Ensure all activities and documentation comply with business processes, regulatory requirements, professional scope, and accreditation standards.
- Participate in workgroups to improve the efficiency, effectiveness, and member-centric focus of wellness coaching services.
What We're Looking For
- A Master's Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, or a related clinical field, OR relevant experience and education determined by the company in lieu of a master’s degree.
- 3 years of experience as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach within a wellness-focused environment such as a health plan, disease management program, or acute care setting.

- Registered Dietitian credential.
- Certified Diabetes Educator (CDE).
- Exercise Physiology certification.
- Certified Lactation Counselor.
- National Diabetes Prevention Program Lifestyle Coach certification.
- Certified Health and Wellness Coach (CHWC).
- Certified Tobacco Cessation Specialist.

Benefits & Compensation
- Compensation range: $50,200.00 - $91,200.00
