About the Role

Responsibilities
- Conduct virtual medical consultations with patients across Pennsylvania
- Deliver comprehensive clinical assessments using telehealth technology
- Develop individualized treatment plans based on patient needs

- Coordinate care with specialists and primary care providers
- Provide preventive health guidance and chronic disease management
- Respond to patient messages within established timeframes

- Support patient education initiatives to improve health outcomes
- Engage in quality improvement activities
- Participate in care team meetings and case reviews
- Ensure compliance with state and federal healthcare regulations
