DaVita Integrated Kidney Care (IKC) is looking for a Full-Time Supportive Care Case Manager (RN). In this remote role, you will provide crucial support for complex patients with CKD/ESKD, helping them navigate the healthcare system. You will autonomously and collaboratively identify medical, social, emotional, and financial needs through review of medical records and implement appropriate interventions to coordinate quality, consistent, and cost-effective care.
What You'll Do
- Support patients with potentially declining conditions by educating them on treatment options and ensuring their care aligns with their wishes.
- Assist patients in identifying their goals, values, and preferences to develop a personalized care plan.
- Work with patients to develop and update advance care plans (ACP).
- Facilitate effective patient and family communication.
- Serve as a role model and coach to help peers facilitate ACP and end-of-life (EOL) conversations.
- Provide comfort case management guidance to other IKC RNs for patients where palliative care is not accessible or who are not ready to stop dialysis but wish to pursue comfort care.
- Coordinate care between the patient's entire care team, including Nephrologists, PCPs, dialysis clinics, and supportive care resources.
- Develop and maintain relationships with supportive care providers.
- Quickly build empathetic, trusting relationships with patients and their families.
- Coordinate all aspects of patient care, including care transitions, management of complex/at-risk patients, and establishing treatment plans in partnership with the care team.
- Ensure streamlined and effective transitions of care when needed, including discharge planning.
- Coordinate with the Operations Manager, Clinical Services Manager, Nurse Practitioners, Social Worker, and other Case Managers to review patient cases and identify clinical judgement escalations.
What We're Looking For
- Minimum of five (5) years experience as an RN.
- Minimum of 2+ years experience as an RN in Hospice or Palliative medicine.
- Current, unrestricted RN License.
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification.
- Intermediate computer skills and proficiency in MS Word, Excel, & Outlook.
- A dedicated home office with internet connectivity of at least 1MB upload and 1MB download speed.
Nice to Have
- BSN degree.
- Previous experience as a DaVita IKC RN Case Manager.
- Residence in the Mountain or Pacific Time zones (Highly Preferred).
- 2+ years of experience in Case Management or Chronic Care Management.
- Minimum of two (2) years experience in renal nursing.
- 2+ years of experience in care plan creation, proactive discharge planning, and utilization.
- Spanish-speaking ability.
- A Compact RN License (Highly Preferred).

Team & Environment
You will coordinate closely with an interdisciplinary team including an Operations Manager, Clinical Services Manager, Nurse Practitioners, Social Workers, and other Case Managers to review patient cases and ensure high-quality care coordination.
