About the Role
The Speech Language Pathologist will evaluate, diagnose, and treat patients with speech, language, and swallowing impairments across various age groups. This role emphasizes patient-centered care in home settings, requiring clinical documentation, family education, and coordination with interdisciplinary teams.
Responsibilities
- Conduct comprehensive assessments of speech, language, and swallowing abilities
- Develop and implement individualized treatment plans for patients
- Provide therapy services in patients' homes and community settings
- Monitor patient progress and adjust treatment strategies as needed
- Document clinical findings and patient outcomes accurately
- Collaborate with physicians, caregivers, and other healthcare providers
- Educate families and caregivers on communication strategies and safety
- Ensure compliance with regulatory and agency documentation standards
- Maintain patient confidentiality in accordance with privacy regulations
- Participate in ongoing professional development and training
- Adapt therapy techniques for diverse patient populations and conditions
- Use clinical judgment to determine appropriate interventions
- Report changes in patient status promptly to supervising clinicians
- Follow infection control protocols during home visits
- Manage assigned caseload efficiently and effectively
- Respond to patient and family inquiries with professionalism
- Contribute to quality improvement initiatives
- Attend team meetings and case conferences as required
- Ensure timely submission of clinical documentation
- Support patient transitions across care settings
