St. Luke's University Health Network is hiring a Social Worker - Rural Health (Master's level) to join our team. This Outpatient Care Manager, Social Worker (OP CM SW) provides Social Work and care management services primarily to outpatients and their families, assessing unique bio-psychosocial statuses, connecting patients with community resources, and collaborating with partners to address social determinants of health. What You'll Do
- Provide assessment, care planning, and intervention, including psychosocial evaluation, advocacy, and crisis intervention.
- Counsel patients and caregivers to help them cope with and understand the impact of illness.
- Consult with providers, nurses, and other healthcare team members to facilitate interdisciplinary care and coordination.
- Investigate insurance benefits and community resources to facilitate appropriate referrals.
- Organize individual patient care meetings with multidisciplinary teams to evaluate progress and resolve problems.
- Provide education to patients, caregivers, or the care team related to government mandates and laws.
- Proactively collaborate with patients, caregivers, and community partners to address bio-psychosocial needs and avoid unnecessary hospitalizations.
- Ensure accurate clinical documentation in patient charts and complete required reports.
- Function autonomously under organizational policies and the NASW Code of Ethics.
- Act as a liaison to community agencies to address systems issues affecting patient outcomes.
- Demonstrate competency in assessment, treatment, and communication appropriate to the patient's age.
What We're Looking For
- Master's degree in Social Work from an institution accredited by the National Council on Social Work Education (NCSWE).
- State licensure for MSW in NJ is required if working in New Jersey.
Nice to Have
- State licensure for MSW in both Pennsylvania and New Jersey.
- MSW with a minimum of two (2) years of experience in medical social work case management or related experience.
Team & Environment
You will collaborate with an Outpatient Care Manager RN, Community Health Worker, and extender staff as needed.
Work Mode
This position operates on a hybrid work model, combining on-site and remote responsibilities.
