Chime is hiring a Social Worker Care Manager (Hybrid - Silver Spring, MD)

Practicebetter is hiring a Social Worker Care Manager to provide vital hybrid care management for patients in the Silver Spring, MD area. This full-time role is primarily remote and telephonic, with occasional travel to patient homes or facilities to directly support mental and physical healthcare.

What You'll Do

• Support the mental and physical healthcare of patients on an assigned caseload.
• Screen and assess patients for common mental health and substance abuse disorders.
• Facilitate patient engagement, follow-up care, and referrals to evidence-based treatments.
• Provide patient education about disorders and available treatment options.
• Systematically track treatment response and monitor changes in clinical symptoms.
• Support psychotropic medication management by consulting with the behavioral health consultant and primary care provider.
• Provide brief behavioral interventions using evidence-based techniques.
• Participate in regularly scheduled caseload consultation with the psychiatric consultant.
• Track patient follow-up and clinical outcomes using a registry and document all encounters.
• Document patient progress and treatment recommendations in the EHR and other required systems.
• Facilitate treatment plan changes for patients not improving as expected.
• Facilitate referrals for clinically indicated services outside the organization.
• Develop and complete relapse prevention self-management plans with patients.
• Utilize Excel and Word, completing assessments and productivity trackers daily.
• Manage time effectively through calendars and the EMR system.

What We're Looking For

• Minimum of an MSW.
• Behavioral Health experience and knowledge.
• Must be a licensed driver with an insured automobile in good working order.
• Ability to communicate effectively, both verbally and in writing.
• Strong time management skills.
