CareHarmony is hiring a Remote Licensed Practical Nurse (LPN) with an active Oregon license for day shift. In this role, you will work comprehensively with providers to deliver value-based care management initiatives, focusing on a resolution-driven approach to improve patient outcomes.
What You'll Do
- Manage a patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
- Identify and coordinate community resources that would benefit patient care.
- Provide patient education and health literacy on the management of chronic conditions.
- Perform medication management, including identifying potential concerns, reconciliation, adherence, and coordinating refills.
- Assist in ensuring timely delivery of services, including Home Health, DME, and Home Infusion.
- Resolve patients' questions and create an open dialogue to understand their needs.
- Assist in managing referrals and appointment scheduling.
What We're Looking For
- Active Oregon LPN License.
- At least 3 years of direct patient-facing work experience.
- Technical aptitude with the Microsoft Office Suite.
- Excellent written and verbal communication skills.
- Must have active high-speed Wi-Fi.
- Must have a home office or HIPAA-compliant workspace.
Nice to Have
- Epic experience.
- Bilingual abilities.
- Additional state LPN licensures.

- Compensation starts at $23/hr ($21/hr + $2/hr OR license bump) with the ability to earn up to $28/hr based on production.
