In this fully remote position, you will work comprehensively with providers to coordinate needs and resources for chronic care management patients, directly impacting their healthcare journey.

What We're Looking For
- Active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license.
- At least 3 years of direct patient-facing work experience.
- Ability to thrive in a fast-paced environment.
- Self-motivated with impeccable attention to detail.

- Compensation: $21/hr.
