CareHarmony is seeking a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to work remotely in a care coordination role. The LPN will collaborate with providers to support value-based care management initiatives for patients, focusing on chronic care management and patient resource coordination.
What You'll Do
- Work comprehensively with providers to deliver value-based care management initiatives for patients
- Identify resources and coordinate needs for chronic care management patients
What We're Looking For
- Must be a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
- At least 3 years of direct patient-facing work experience
- Must thrive in a fast-paced environment
- Must be self-motivated
- Must have impeccable attention to detail
- Must value the impact they can have on a patient’s healthcare journey
