A Remote Hospice Triage Registered Nurse (RN) is needed at IntellaTriage to provide after-hours phone support for hospice patients and families, ensuring timely interventions and care coordination. This remote role serves as an extension of client care teams during non-core hours, delivering compassionate, patient-centered care through best-in-class technology.
What You'll Do
- Provide critical after-hours support to hospice patients and families over the phone
- Triage hospice patient and family needs with professionalism and empathy
- Ensure timely interventions and coordination of care for hospice patients
- Serve as an extension of clients’ care teams during non-core hours
- Use client-customized protocols to deliver patient-centered, compassionate care

Work Mode
