Rockefeller Philanthropy Advisors is hiring a Project Coordinator for Case Management in Allegheny County. In this role, you will coordinate essential services and resources for individuals striving to live independently. You will collaborate directly with program participants and project teams to assess needs, implement initiatives, and ensure positive community outcomes.
What You'll Do
- Assess, identify, and secure services tailored to participants’ unique needs while monitoring progress.
- Support the Project Manager in developing and executing new programs and initiatives.
- Respond promptly to participant requests and facilitate access to adaptations, medical equipment, transportation, and other resources.
- Ensure compliance with all documentation, assessments, service plans, and reporting requirements within established timeframes.
- Maintain detailed, person-centered records of visits and contacts using required systems.
- Consistently demonstrate organizational values in all professional interactions and duties.
What We're Looking For
- Bachelor’s degree.
- Minimum three years of case management experience.
- Strong communication, customer focus, integrity, and problem-solving skills.
- Proficiency with relevant computer systems and technology.
- Physical ability to perform duties including reaching, stooping, walking, standing, and lifting up to 20 pounds.
- Valid driver’s license and reliable transportation to travel throughout Allegheny County and surrounding areas.
Nice to Have
- Bachelor’s degree in fields such as sociology, social welfare, psychology, gerontology, or criminal justice.
- Case management experience within the PA Office of Long Term Living.
