Creative Solutions in Healthcare is hiring a Full-Time Physical Therapist to join our in-house rehabilitation team. In this role, you will provide person-centered care to residents, focusing on developing individualized treatment plans and working closely with an interdisciplinary team.
What You'll Do
- Consult with patients to learn about their physical conditions.
- Assess and interpret patient evaluations and test results.
- Develop treatment plans using a variety of techniques.
- Create personalized rehabilitation programs for patients.
- Administer medically prescribed physical therapy treatments to relieve pain, improve function, and promote healing.
- Ensure all delivered treatment follows the established plan of care.
- Monitor patient response to treatment and modify the approach as needed with IDT collaboration.
- Provide clinical support and education to patients, family members, and caregivers.
- Collaborate with other interdisciplinary team members to ensure quality patient care.
- Ensure documentation is completed thoroughly and timely, complying with all company and regulatory requirements.
- Comply with all rules, regulations, and procedures.
- Attend required meetings as designated by the Director of Rehab.
What We're Looking For
- An active and valid Texas license in Physical Therapy.
- In good standing with all regulatory agencies and licensing boards.
- Current knowledge of treatment practices.
- Ability to manage patients with different types of personalities.
Nice to Have
- SNF or long-term care experience.
- Knowledge of the Net Health Electronic Documentation System.

You will be part of an in-house rehab team and work within an interdisciplinary team structure, reporting to the Director of Rehab.
