Providence Health & Services at Mission Hospital Regional Medical Center is looking for a Physical Therapist to provide skilled therapy services to patients in their homes. This role delivers whole-person care in accordance with physician plans, focusing on individual patient needs to promote maximum independence.
What You'll Do
- Manage and provide skilled physical therapy to patients/clients in a home setting.
- Perform patient/client evaluation and treatment.
- Conduct professional consultations to achieve maximum patient/client independence.
What We're Looking For
- Bachelor's, Master's, or D.P.T. Degree from an accredited Physical Therapy Program.
- National Provider BLS - American Heart Association within 30 days of hire.
- Oregon Physical Therapist License upon hire.
- 1 year of Clinical experience in Home Health or a healthcare setting, or active enrollment in the Providence postgraduate preceptor program.

This is an onsite position providing care in a Home Health setting in Oregon.
