The Personal Service Coordinator II plays a vital role in supporting individuals with mental health challenges by providing comprehensive case management and personalized care coordination. This position is responsible for conducting in-depth client assessments, developing recovery-oriented service plans, and connecting clients with essential community resources. The coordinator ensures continuity of care through regular follow-ups, home visits, and collaboration with multidisciplinary teams. Emphasis is placed on promoting client independence, improving quality of life, and fostering long-term stability within a trauma-informed and client-centered framework. This role operates with a high degree of autonomy while adhering to regulatory standards, ethical guidelines, and organizational policies.
Responsibilities
- Perform initial intakes and conduct thorough assessments of clients' life circumstances, identifying challenges and support needs.
- Collaborate with clients to develop personalized care plans with their input and consent.
- Support clients in creating and following through on individualized care strategies.
- Connect clients with community resources and facilitate access to needed services.
- Monitor the effectiveness of support services, tracking participation from both clients and providers.
- Deliver consistent client care through regular contact, including home visits as needed.
- Offer direct, proactive support to address client challenges, providing guidance and practical solutions.
- Keep accurate daily logs of activities and client interactions.
- Complete progress documentation and billing forms according to organizational and contractual guidelines.
- Ensure adherence to Short Doyle and Medi-Cal case management regulations.
- Meet established monthly productivity benchmarks based on required service hours.
- Build and sustain professional relationships with external agencies and service providers.
- Uphold HIPAA compliance, ethical standards, and appropriate professional boundaries.
- Operate independently and make informed decisions with minimal supervision.
- Transport clients when necessary, following program protocols.
- Use personal vehicle for work-related travel as required.
- Participate in after-hours on-call responsibilities as assigned.
- Conduct mental health evaluations under LPS sections 5585 or 5150 when authorized.
- Perform additional duties as needed to support program operations and organizational goals.
Requirements
- Bachelor’s degree in human services or a related field from an accredited institution.
- Four years of relevant professional experience in human or mental health services.
- Graduate education of up to two years may substitute for equivalent experience on a one-to-one basis.
- Post-associate clinical experience of up to two years may count toward educational requirements, in addition to four years in mental health settings.
- Valid California Driver’s License and proof of auto insurance.
- Documentation of educational credentials.
- Eligibility to bill for mental health services, excluding psychotherapy, including unplanned and targeted case management within scope of practice.
- Strong organizational abilities and meticulous record-keeping skills.
- Effective interpersonal communication abilities.
- Proficient written and verbal communication skills.
- Intermediate computer proficiency using PC systems.
- Capacity to work autonomously and make sound, independent judgments.
- Ability to transport clients using personal vehicle when necessary.
- Knowledge of rehabilitation models may be required.

- Frequent sitting with occasional standing and walking during work hours.
- Occasional lifting of up to 20 pounds may be necessary.
