About the Role
This position supports patients by coordinating care activities over the phone, guiding individuals through healthcare services, monitoring treatment plans, and connecting them with appropriate resources to improve health outcomes.
Responsibilities
- Conduct initial patient assessments via telephone to identify care needs
- Schedule medical appointments and coordinate referrals across providers
- Monitor patient progress and adherence to treatment plans
- Provide education on disease management and preventive care
- Document all patient interactions in electronic health records
- Follow up with patients to ensure continuity of care
- Assist patients in navigating insurance and billing inquiries
- Escalate complex cases to clinical supervisors as needed
- Maintain compliance with privacy and confidentiality standards
- Utilize call center software and healthcare information systems efficiently
- Respond to patient inquiries with empathy and accuracy
- Support care transition processes from hospital to home
- Coordinate with social workers and community resources
- Track patient outcomes and report data as required
- Participate in team meetings and quality improvement initiatives
- Adhere to established call handling and resolution protocols
- Ensure timely communication between patients and care teams
- Assist in patient outreach for preventive screenings
- Help patients understand medication regimens and instructions
- Maintain up-to-date knowledge of available health programs
