Responsibilities
- Reach out to new patients using phone, text, video, or in-person methods to link them with medical services and care team referrals.
- Build trust and increase patient involvement through compassionate and reliable communication.
- Gather and assess patient information from electronic records and other sources to support care monitoring, prioritize needs, and promote preventive services.
- Exchange updates with primary care providers using EHR messaging and in-person check-ins.
- Act as the main point of contact for healthcare facility providers, maintaining consistent and cooperative relationships.
- Join weekly care team meetings and work alongside pharmacists, social workers, and community health workers to ensure coordinated care.
- Record detailed and accurate notes for every patient interaction.
- Work with internal support teams to evaluate current processes and help design more effective workflows for patient care.
