Salud Family Health is hiring a full-time Nurse Practitioner or Physician Assistant to provide integrated healthcare in Fort Morgan, Colorado. In this role, you will work within a team-based medical home model, delivering comprehensive care focused on our mission of serving the community.
What You'll Do
- Perform routine clinic operations.
- Take patient histories and conduct physical assessments.
- Work in a team-based care fashion that supports patients and families in self-management, self-efficacy, and behavior change.
What We're Looking For
- Graduation from an accredited Nurse Practitioner or Physician Assistant program.
- An unrestricted Physician Assistant license or Nurse Practitioner license in good standing in the State of Colorado.
- Sensitivity to low-income and ethnic minority communities.
Nice to Have
- 3 years of experience in a clinical setting as a Nurse Practitioner or Physician Assistant.
- The ability to speak Spanish.

- Compensation range: $105,000 to $138,750.
