DaVita IKC is seeking a Nurse Practitioner to provide holistic, integrated care for patients with complex kidney conditions. In this role, you will be a vital part of a mission-driven team dedicated to reducing hospitalizations and improving patient outcomes. Your primary focus will be completing Comprehensive Health Evaluations (CHEs) to manage comorbid conditions and address a full spectrum of patient needs.
What You'll Do
- Complete Comprehensive Health Evaluations (CHEs) to assess and manage comorbid conditions.
- Manage CKD and ESKD patients, focusing on slowing disease progression, reducing costs, and preventing readmissions.
- Utilize prescriptive authority, medication management, and diagnostic interpretation in collaboration with nephrologists and interdisciplinary teams.
- Lead the clinical team to ensure care coordination aligns with medical treatment plans and addresses both medical and psychosocial needs.
What We're Looking For
- Passion for caring for patients with complex, chronic illnesses and a willingness to adapt as the program evolves.
- A minimum of 2 years of experience as a Nurse Practitioner (NP).
- Current, unrestricted NP license in state of practice; DEA license or eligibility within 90 days.
- Current CPR/BLS certification, valid driver’s license, and insurance in the state.
- Intermediate computer skills (MS Word, Excel, Outlook).
- Strong clinical judgment, autonomous decision-making, and expertise in managing complex comorbidities.
- Positive, solution-focused attitude, committed to improving patient and team outcomes.
- A home office setup with high-speed internet and reliable transportation.
Nice to Have
- Experience in Value-Based Care or Population Health, including Medicare Advantage documentation and Health Risk Assessments.
- 5+ years of experience as a Family NP, Primary Care NP, or in Internal Medicine, Cardiology, Nephrology, or Endocrinology.
- Experience managing population health with a focus on reducing hospitalizations.
- Prior experience with Cerner or similar EHR systems.

- Compensation: $118,000.00 - $150,000.00 per year.

Work Mode
This is a hybrid-remote position based in the DC area, covering an assigned geographic area that includes dialysis clinics and nephrology practices in locations such as Baltimore, MD.
