About the Role
The Nurse Practitioner will deliver high-quality clinical care to patients with chronic illnesses, primarily kidney disease. This role involves close collaboration with physicians and care teams, using evidence-based practices to optimize patient outcomes. The hybrid remote model supports both virtual and in-person patient interactions.
Responsibilities
- Deliver comprehensive medical evaluations and ongoing care for patients with chronic conditions
- Collaborate with physicians and care teams to develop individualized treatment plans
- Conduct virtual and in-person patient visits as part of a hybrid schedule
- Monitor patient progress and adjust care strategies based on clinical data
- Order and interpret diagnostic tests to inform treatment decisions
- Prescribe medications and manage therapeutic regimens
- Educate patients and families on disease management and preventive care
- Maintain accurate and timely electronic health records
- Ensure compliance with clinical protocols and regulatory standards
- Participate in quality improvement initiatives
- Support patient engagement through consistent communication
- Respond to patient concerns and urgent clinical needs
- Coordinate care transitions across healthcare settings
- Promote adherence to treatment plans through motivational strategies
- Use telehealth platforms to deliver remote care services
- Attend team meetings and case reviews
- Stay current with clinical guidelines and best practices
- Contribute to patient safety and risk mitigation efforts
- Support clinical training and onboarding for new staff
