Brave Health is looking for a Medical Records Specialist to manage the secure and compliant flow of patient health information. You will ensure patient data is accurate, complete, and securely maintained, supporting seamless access for patient care, audits, and other requests.
What You'll Do
- Evaluate, prioritize, and process incoming and outgoing records requests.
- Verify and review forms for accuracy, legibility, and completeness, ensuring current and valid Release of Information forms are on file.
- Maintain the medical records tracker and ensure records are up to date.
- Conduct quality checks to ensure the accuracy, confidentiality, and HIPAA compliance of all records.
- Adhere to all HIPAA and confidentiality policies.
- Interface with patients and external partners to ensure a high level of customer experience.
- Gather and deliver patient Letters of Service, progress updates, and other requested documents.
- Draft documentation requests for supervisor approval.
- Maintain responsibility for client and partner satisfaction through all stages of the medical records operations process.
- Process and review patient documents for Release of Information, document retrieval, and audits.
- Respond promptly to external medical records audit requests, ensuring the appropriate format and compliance.
- Liaise with third-party requestors, including law enforcement, providers, attorneys, and other entities.
- Secure new Release of Information forms for expired requests as needed.
What We're Looking For
- High school diploma or equivalent.
- 2+ years of experience working in medical records.
- Strong understanding of medical terminologies and records management processes.
- Proficiency in ICD-10-CM coding standards, CPT-4 terminologies, and HCPCS coding.
- Familiarity with client-specific process rules, regulatory requirements, and HIPAA compliance.
- Strong attention to detail and analytical skills to identify errors or inconsistencies in documentation.
- Excellent time management and workload prioritization abilities.
- Demonstrated professionalism in written and verbal communication.
- Proficiency in using electronic health record systems, document management software, and working in remote environments.
- Tenacity, problem-solving, and the ability to work effectively under pressure.
- Extensive knowledge of HIPAA regulations and confidentiality policies.
- Ability to ensure accuracy and security of all patient data.
Nice to Have
- Experience in behavioral health or virtual healthcare environments.
- Associate’s degree or certification in Health Information Management.
