Requirements
- Provide resources and medical telephonic coaching, information, and educational materials to patients.
- Provide clinical support to CommUnityCare Provider teams.
- Act as a liaison between the patient and the health centers.
- Direct patient flow to ensure patients disposition is accurate as needed.
- Assist patients with securing appropriate medical care and services within the health system and community and facilitate patient access to medical home providers, staff and resources.
- Accurately and promptly schedules, reschedules, and cancels physician and non-physician patient appointments.
- Identify urgent scheduling needs and prioritize them with the appropriate health center or resource.
- Identify urgent and potentially urgent patient care situations and escalate as appropriate for a positive patient outcome.
- Participate in orientation of new employees and attend mandatory meetings.
- Work as a team member which involves interacting with staff, patients, and providers to communicate and coordinate patient care and workflow effectively.
- Perform other duties as assigned.

- Strong attention to detail and accuracy.
- Excellent verbal and written communication skills.
- Demonstrated familiarity with tools, technology, and systems typically found within most health care environments (i.e. personal computer skills, spreadsheets, word processing, patient records systems, EMR systems, etc.)
