PurposeCare is looking for a Licensed Practical Nurse to join our home health team. You will provide skilled visits and personalized, holistic care directly in patients' homes, focusing on real-life therapy to help them overcome challenges and make meaningful progress in their daily lives and recovery.
What You'll Do
- Provide skilled visits that address the full scope of a patient's needs in their home.
- Build meaningful, one-on-one connections with patients in their own homes.
- Make clinical decisions independently and use your expertise to its fullest.
What We're Looking For
- Current LPN license with your state Board of Nursing.
- 1 year of nursing experience.
- Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing.
- Current Driver's License, vehicle insurance, and/or access to dependable transportation.
- Ability to successfully complete a background check.
